Left ventricular hypertrophy in diabetes mellitus: Is it sympathetically mediated?.

Sympathetic stimulation is a well known factor in the pathogenesis of left ventricular hypertrophy (LVH) in patients with hypertension out its contribution to the development of LVH among diabetic hypertensives who are presumably with dysautnomia is unknown. To this end, we examined the autonomic function using heart rate variability (HRV) of 57 patients, (35 males and 22 females) with mean age of 52. The clinical data and the 2 D-echocardiogram were reviewed. Patients were classified into three groups, namely: Group 1 (21): those without LVH or DM; Group 2 (15): those with LVH no without DM; and Group 3 (21): those with both LVH and DM. Results showed that non-diabetics (group I and II) and lower sympathetic activity than the diabetics. Group II had a significantly lower sympathetic activity than group I (t=3.0357; 95 percent level of confidence). The sympathetic activity of Group III was significantly lower than that seen in groups I and II (t=15.83 and 13.82, respectively). Group III did not show any circadian pattern. There was exaggerated parasympathetic activity among group III patients compared to groups I and II. Our findings suggest that stimulus for LVH development in DM does not operate on increased sympathetic tone, where it is actually diminished. The exaggerated parasympathetic activity in this group of patients may at least in part contribute to LVH genesis.
