Background: Carotid artery stenosis have a role in the development of stroke after CABG. Awareness of such disease will reduce the incidence of stroke if carotid artery carotid artery stenosis in patients with stenosis was investigated pre operatively.Open heart surgery in patients without carotid atherosclerosis have increased risk of stroke but the risk increases if significant CAS is present. There were reported racial difference in the severity and distribution of carotid atherosclerosis.1 In the Philippines, the prevalence of carotid artery stenosis is unknown. There was no reported local study yet showing the association of CAD with CAS in our population.
Objectives: To determine the prevalence of carotid artery stenosis in patients with coronary artery disease confirmed by coronary angiogram in the PHC setting.
Methods: Study population consisted of 156 patients who underwent coronary angiogram for suspected CAD. Concomitant carotid scan was done in these patients evaluate the carotid arteries for the presence of plaques.
Results: Out of 156 patients studied, 128 (82%) were males and 28 (18%) were females. Mean ± SD age of patients studied was 6.1± 9.1 (range 38-81) years. The overall prevalence of internal carotid artery stenosis was 124 (79%) while coronary artery disease prevalence was 141( 90%). Internal carotid artery stenosis of >50% with coronary artery disease was found in 17(11.0%) with overall prevalence of 74%. Severity of internal carotid artery stenosis is significantly correlated with severity of coronary artery disease with p value =0.00. Significant stenosis (≥50%) was seen in 17 (11.0%) of patients; 7 (4.6%) of which had critical stenosis (≥70%); 2 (1.3%) had complete occlusion of the left internal carotid artery. Diabetes and age were shown to
have significant correlation with internal carotid artery stenosis.
Conclusion: The frequency of carotid artery stenosis in our patients is similar to other reports. Severity of internal carotid artery stenosis is significantly correlated with severity of coronary artery disease with p value =0.00. Age and presence of diabetes are important factors for carotid artery stenosis in patients for coronary artery bypass grafting.
